Immunizations Requirements for School Attendance
A validated document of
immunization (one issued by a public health clinic or signed by a licensed
physician) must include the following:
PRE-KINDERGARTEN (4 YEARS OF AGE)
- four doses of DPT (does
not have to be after fourth birthday); fifth dose needed at age 5
before entering kindergarten if fourth dose is given before the fourth
birthday
- three doses of Polio
(does not have to be after fourth birthday)
- two MMRs - The second
one can be a measles vaccine.
- HIB (three doses with
one given after 12 months, two doses between 12-15 months, or one dose
after 15 months)
- three doses of Hepatitis
B (for children who were born on or after Sept. 2, 1992)
- one dose of Varicella
(chicken pox) for students born on or after Sept. 2, 1994
- Cypress-Fairbanks ISD is
requesting that parents present proof of updated immunizations to the
school nurse at the beginning of the semester their children turn
five.
KINDERGARTEN (AGE 5 THROUGH
6 YEARS)
- four doses of DPT (last
one must be after fourth birthday)
- three doses of Polio
(last one must be after fourth birthday)
- two MMRs - The second
one can be a measles vaccine.
- three doses of Hepatitis
B (for students born on or after Sept. 2, 1992)
- one dose of Varicella
(chicken pox) for students born on or after Sept. 2, 1994
AGE 7 AND OLDER
- three doses of DPT (last
one on or after fourth birthday)
- three doses of Polio
(last one on or after fourth birthday)
- one MMR
- second measles (students
born on or after Sept. 2, 1991)
- three doses of hepatitis
B (students born on or after Sept. 1, 1992)
AGE 12 AND OLDER
The following vaccines are
required in addition to those above.
- second measles vaccine
(at the beginning of the semester the student turns 12)
- three doses of Hepatitis
B (students born between Sept. 2, 1988, and Sept.1, 1992)
- one dose of Varicella
(students born between Sept. 2, 1988, and Sept. 1, 1994)
- two doses of Varicella
if student was not vaccinated before 13th birthday
- Td Booster within the
last 10 years
- Cypress Fairbanks ISD is
requesting that parents present proof of updated immunizations to the
school nurse at the beginning of the semester their children turn 12.
Parent or physician
statements of prior Varicella illness will be allowable to substitute for
vaccination.
